Parent/Guardian consent form is required for non-confidential services.
Eligibility
The SHACC serves students enrolled in Sault Area High School and Career Center, Sault Area Middle School and Malcolm School, without regard to ability to pay, insurance status, race, religion, color, national origin, creed, disability, age, gender identity, sex, sexual orientation, or sexual
preference.

Additional Services
The SHACC will provide limited, non-emergency medical services to staff members if openings are available.
However, students take priority for scheduled and walk-in services.
Treatment for minor acute illness or immunizations will be billed to the adult if health insurance does not coverage service. This includes copays and deductibles. Treatment for chronic illness or health promotion exams are not provided to adults.
Insurance including Medicaid is accepted!

$10 / Comprehensive Physical Examination
$10 / each Immunization
$15 / TB Test

A sliding fee scale is available and is based on student's income.
No student is denied services based on inability to pay!
Students without insurance are encouraged to apply for Medicaid.
